Transition Team - Licensed Practical Nurse (Private Duty)

📋 Description

• FT – 40 hours/week; transition care at home.
• Care for infants, children, and adults at home.
• Serve Chester, Delaware, and Montgomery Counties.
• Transition care from hospital to ongoing home care.
• Learn to care for clients with rare and complex diseases.
• Deliver quality care and training to the ongoing team.

🎯 Requirements

• Have a current license as a Registered Nurse / RN
• NICU, PICU, ICU, or home care trach/vent experience required
• Achieve a passing score on high-tech Adult and Pediatric simulations.
• Hold current CPR certification
• Availability to commit to an ongoing, consistent schedule
• Ability to travel throughout Chester, Delaware, and parts of Montgomery County
