Transition Team - Licensed Practical Nurse (Private Duty)

📋 Description

• Deliver quality home care to clients on the Transition Team
• Support infants, children, and adults with complex needs at home
• Facilitate care transition from hospital to home care
• Learn to care for clients with rare and complex diseases
• Provide extensive training to the BAYADA care team
• Choose day or overnight shifts (Mon-Sun)

🎯 Requirements

• Have a current license as a Registered Nurse / RN
• NICU, PICU, ICU, or home care trach/vent experience required
• Achieve a passing score on high-tech Adult and Pediatric simulations
• Hold current CPR certification
• Availability to commit to an ongoing, consistent schedule
• Ability to travel throughout Chester, Delaware, and parts of Montgomery County
