Transitional Care Manager

📋 Description

• Develop and sustain strong partnerships with referral sources, physicians, and other health care
• Clearly explain the features and benefits of BAYADA services to referral partners and clients
• Apply knowledge of clinical and regulatory home care eligibility criteria to every referral
• Coordinate with the service office to set up physician-ordered services quickly and accurately
• Stay organized

🎯 Requirements

• Motivated, self-starting mindset with a passion for advocating for patients
• Hungry, humble, and smart
• Proven strength in interpersonal skills
• Working knowledge of the Medicare Conditions of Participation (COPs)
• At least one year of health care experience
