📋 Description

• Remotely monitor patients using advanced telemonitoring technology to promote safety, well-being
• Maintain continuous observation of patients with identified safety concerns and promptly recognize
• Communicate virtually with patients to provide reassurance, support, assistance, and appropriate
• Communicate significant changes or concerns to appropriate clinical teams and escalate issues
• Collaborate with interprofessional healthcare teams to support timely intervention, de-escalation
• Complete accurate and timely documentation of patient observations, communications, and

🎯 Requirements

• Secondary School Diploma
• Bachelor’s degree in health sciences or related field (preferred)
• 2–4 years experience in customer service, patient-facing, or front-facing roles
• 1–3 years experience in healthcare environment (preferred)
• Strong verbal and written English communication skills
• Excellent observation and attention to detail
