Weekend Admissions Physical Therapist

📋 Description

• Complete OASIS Start of Care (SOC) admissions for adult/geriatric home health.
• Assess mobility, strength, balance, gait, endurance and safety.
• Develop individualized treatment plans per patient needs and orders.
• Apply therapies and recommendations to support recovery.
• Collaborate with nursing and interdisciplinary team members.
• Educate patients and families on rehab and safe mobility techniques.
• Continuously assess status and revise care plans as needed.
• Document evaluations and services using EMR and tablet tech.

🎯 Requirements

• Current New Jersey Physical Therapist (PT) license.
• Minimum of one year of recent experience as a licensed Physical Therapist.
• Home health experience required.
• OASIS experience required.
• HCHB experience strongly preferred.
• Excellent assessment, communication, and time management skills.
• Ability to work independently while providing exceptional patient care.
