Weekend Admissions Physical Therapist

📋 Description

• Complete OASIS SOC admissions visits for adult/geriatric patients.
• Conduct PT evaluations: mobility, strength, balance, gait, endurance.
• Develop individualized treatment plans per patient needs and orders.
• Implement therapy interventions to support recovery and independence.
• Collaborate with nursing and interdisciplinary team.
• Educate patients and families on rehab strategies and safe mobility.

🎯 Requirements

• Current New Jersey Physical Therapist (PT) license.
• Minimum of one year of licensed PT experience.
• Home health experience required.
• OASIS experience required.
• HCHB experience strongly preferred.
• Excellent assessment, communication, and time management skills.
