Weekend Baylor Physical Therapist, PT, Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform mobility, ROM, strength, balance, and safety assessments.
• Implement wound prevention and wound care interventions.
• Develop and update individualized PT care plans (manual therapy, gait).
• Educate clients and families on rehabilitative care and safety.
• Document observations and interventions on a touchpad EMR.

🎯 Requirements

• PA Physical Therapist (PT) license required.
• Minimum 1 year PT experience.
• Graduation from APTA-approved PT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, problem-solving skills.
• Solid computer skills; EMR experience preferred.
