📋 Description

• Make home visits to clients in designated territories.
• Perform mobility, strength, balance, and safety assessments.
• Implement wound care and evidence-based dressings.
• Develop and adjust individualized PT care plans and interventions.
• Continually assess and revise the care plan with nursing.
• Educate clients and families to promote health and independence.

🎯 Requirements

• AZ Physical Therapist license.
• Minimum one year PT experience.
• Graduation from APTA/AMA-accredited program (transcript/diploma).
• Ability to work independently and manage time.
• Strong interpersonal and organizational skills.
• Proficient computer skills; EMR experience preferred.
