Weekend Physical Therapist (PT)- Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements: mobility/ROM, transfers, balance, ambulatory status.
• Implement wound care with evidence-based dressings and topical agents.
• Develop individualized care plans including manual therapy, gait training, and balance.
• Continually assess and revise the PT care plan; coordinate with nursing.
• Educate clients and families to promote safety and independence; document in EMR.

🎯 Requirements

• A current Pennsylvania Physical Therapist (PT) license.
• Minimum of one year of recent PT experience.
• Graduation from a CAPTE/APTA-accredited program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
