Weekend Physical Therapist (PT)- Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests: ROM, mobility, gait, strength, safety.
• Implement wound care interventions using evidence-based dressings/topical agents.
• Develop and implement individualized care plans: manual therapy and gait training.
• Assess and revise PT care plan; collaborate with nursing in the multidisciplinary team.
• Educate clients and family on rehabilitative care for health and independence.

🎯 Requirements

• PA PT license (current).
• Minimum 1 year PT experience.
• Graduation from APTA program or AMA CAHEA accreditation.
• Ability to work independently and manage time.
• Strong interpersonal/organizational/problem-solving skills.
• Solid computer skills; EMR experience preferred.
