📋 Description

• Make home visits to clients in designated geographic territories.
• Perform mobility and balance assessments and strength testing.
• Wound care interventions and dressings.
• Develop individualized PT care plans with manual therapy and gait training.
• Continually assess and revise PT plan with nursing.
• Educate clients and families in rehab care and safety.

🎯 Requirements

• Current PT license (state).
• Minimum 1 year PT experience.
• Graduate of APTA-approved program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, problem-solving skills.
• Solid computer skills; EMR experience preferred.
