Wound, Ostomy, and Continence Nurse (WOCN)

📋 Description

• See clients from hospital discharge to safe home life.
• Deliver 1:1 skilled nursing and wound care per care plan.
• Conduct wound care consultations and provide oversight.
• Perform assessments and educate patients and families; monitor condition.

🎯 Requirements

• Active and current PA RN license.
• WOCN certification required.
• At least one year of nursing experience.
• Prior home care experience strongly preferred, but not required.
• Ability to travel to cases as assigned.
• Calm, reassuring presence with active listening, strong assessment, and communication skills.
• Ability to deliver patient care with compassion, excellence, and reliability.
