Home Health Registered Nurse - RN Preceptor

📋 Description

• Train, coach, and mentor newly hired RNs and LPNs
• Maintain a caseload and patient care responsibilities
• Use strong head-to-toe assessment skills to identify subtle changes in patient condition
• Utilize experience with acute and post-acute care (e.g., CHF, COPD, wound care, IV therapy
• Triage and manage urgent clinical situations independently in the home setting
• Perform medication reconciliation, teaching, and safety monitoring

🎯 Requirements

• Current nursing license in good standing in the state of Colorado
• Graduation from an accredited and approved nursing program, as indicated by school transcript or
• At least one year of current or recent hands-on medical experience within the past two years with
• Previous Medicare Certified Home Health and OASIS documentation experience is required
• Ability to work independently while knowing when to escalate concerns
• Comfortable making real-time clinical decisions without immediate on-site support
