Home Health Visits Physical Therapist

📋 Description

• Make home visits to clients in designated geographic territories
• Perform diagnostic tests and measurements, such as mobility, strength, and balance
• Implement wound prevention and repair interventions using evidence-based dressings
• Develop and implement individualized care plans including therapeutic exercises
• Assess and revise PT care plan and collaborate with nursing team
• Educate clients and families on rehabilitative care and safety

🎯 Requirements

• Current Maryland Physical Therapist license
• Graduation from an APTA or AMA approved program
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills
• Solid computer skills; EMR experience preferred
