Medical Social Worker, Home Health

📋 Description

• Making home visits to clients in designated geographic territories
• Assisting the team in understanding social and emotional factors related to clients' health problems
• Providing clinical social evaluations and planning appropriate interventions
• Identifying community resource referrals for clients and families
• Acting as an advocate for clients and families to navigate community systems
• Participating in development of total plan of care with multidisciplinary team

🎯 Requirements

• Licensed by the Pennsylvania Board of Social Work Examiners at Master (LMSW) or Independent (LISW)
• Master's degree in Social Work with at least 2 years of community-based experience
• Accreditation by the Council on Social Work Education
• Home care experience preferred
• Ability to read, write, and communicate effectively in English
• Ability to work independently and manage time effectively
