Physical Therapist

📋 Description

• Perform skilled physical therapy visits throughout designated territories.
• Complete comprehensive physical therapy evaluations and ongoing reassessments.
• Assess mobility, strength, balance, gait, transfers, endurance, range of motion, and overall
• Develop and implement individualized treatment plans using evidence-based interventions, including
• Implement skin integrity interventions to help prevent wound development and promote healing when
• Collaborate with nurses and interdisciplinary team members to coordinate exceptional patient care.

🎯 Requirements

• Current New Jersey Physical Therapist (PT) license.
• Minimum of one year of recent experience as a Physical Therapist.
• Graduate of an accredited Physical Therapy program approved by the American Physical Therapy
• Ability to work independently and effectively manage time.
• Strong communication, organizational, and clinical assessment skills.
• Experience with electronic medical records (EMR) preferred.
