Visit a variety of clients in their own home within designated geographic areas.
Perform diagnostic tests and measurements (mobility, ROM, gait, balance, endurance, safety).
Implement protective interventions to prevent wounds and promote healing with evidence-based
Develop and implement individualized care plans, including manual therapy, gait training, and
Continuously assess and revise the PT care plan; collaborate with nursing on multidisciplinary
Educate clients and families to promote health, safety, and independence.

🎯 Requirements

Active and current Physical Therapist license.
One year of clinical experience preferred.
Home health care experience welcomed; training provided.
Ability to travel locally to cases as assigned.
