Physical Therapist (PT) – Home Health Visits

📋 Description

• Provide one-on-one home health visits for adult and geriatric clients throughout Hudson County, NJ
• Perform skilled physical therapy visits throughout designated territories
• Complete comprehensive physical therapy evaluations and ongoing reassessments
• Assess mobility, strength, balance, gait, transfers, endurance, and overall functional status
• Develop and implement individualized treatment plans using evidence-based interventions
• Provide therapeutic exercise, gait training, balance training, manual therapy techniques, and

🎯 Requirements

• Current New Jersey Physical Therapist (PT) license
• Graduate of an accredited Physical Therapy program
• Home health experience preferred, but not required
• Strong communication, organizational, and clinical assessment skills
• Ability to work independently and effectively manage a flexible schedule
• Experience with electronic medical records (EMR) preferred
