Physical Therapy Assistant, Home Health

📋 Description

• Provide skilled physical therapy services through home visits within designated territories.
• Perform comprehensive evaluations, including assessments of mobility, strength, balance, gait
• Develop individualized treatment plans based on patient needs and physician orders.
• Implement evidence-based therapeutic interventions, including therapeutic exercise, gait training
• Provide integumentary and wound prevention interventions as clinically appropriate.
• Collaborate with nursing and interdisciplinary team members to coordinate patient care.

🎯 Requirements

• Current New Jersey Physical Therapist (PTA) license.
• Minimum of one year of recent clinical experience as a licensed Physical Therapist Assistant.
• Graduate of an accredited Physical Therapy Assistant program.
• Ability to work independently and effectively manage time.
• Strong communication, organizational, and problem-solving skills.
• Electronic medical record (EMR) experience preferred.
