Registered Nurse, Home Health Visits

📋 Description

• Use strong head-to-toe assessment skills with the ability to identify subtle changes in patient
• Utilize experience with acute and post-acute care (e.g., CHF, COPD, wound care, IV therapy
• Triage and manage urgent clinical situations independently in the home setting
• Perform medication reconciliation, teaching, and safety monitoring
• Perform comprehensive wound assessment, care, and documentation
• Manage a dynamic visit schedule and prioritize urgent visits

🎯 Requirements

• Current nursing license in good standing in the state of Pennsylvania
• Graduation from an accredited and approved nursing program (transcript/diploma)
• At least one year of current/recent hands-on medical experience within the past two years with
