📋 Description

• Making home visits to hospice clients within your assigned geographic territory.
• Providing skilled nursing care, including medication administration, wound care, treatments
• Assessing patients' conditions and communicating changes to the appropriate clinical team members.
• Implementing and documenting physician orders accurately and promptly.

🎯 Requirements

• Current Registered Nurse (RN) license in Vermont and New Hampshire.
• At least one year of recent, verifiable clinical experience in medical-surgical nursing or a

• Basic computer skills; electronic medical record (EMR) experience preferred.
• Ability to travel to patient homes within the assigned service area.
