📋 Description

• Conduct comprehensive in-home client assessments and complete the care planning process, including
• Develop, implement, and update individualized care plans that reflect each client’s goals
• Provide ongoing clinical and case management oversight for an assigned caseload, ensuring services
• Collaborate with physicians and other health care providers to coordinate care, obtain and document

🎯 Requirements

• Graduate of an accredited school of nursing.
• Current Registered Nurse license in good standing in the state of Arizona.
• Two or more years of recent clinical experience preferred, particularly in home care
• New graduates and newly licensed Registered Nurses are encouraged to apply.
• Experience with client assessments, care planning, clinical documentation, caregiver supervision
