📋 Description

• Coordinate client services and respond quickly to scheduling needs and changes
• Manage and proactively grow your client caseload
• Deliver an exceptional experience for clients, families, caregivers, and referral partners
• Build strong relationships with referral sources, payors, and community organizations throughout
• Partner closely with Clinical Managers to support and supervise field employees, including HHAs and
• Monitor key performance and financial metrics for your caseload

• Bachelor's degree
• Strong communication, relationship-building, and organizational skills
• Comfort working with technology and electronic medical record systems
• Demonstrated success taking on increased responsibility and achieving goals
