Make home visits to clients in designated geographic territories
Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions
Develop treatment plans based on evaluation and physician's orders
Continually assess and revise speech therapy care plan
Educate patients and family members on rehabilitative care
Accurately document observations and interventions using a touchpad tablet

🎯 Requirements

Current license as a Speech Language Pathologist in Vermont
Certificate of clinical competence in speech from ASHA or equivalent
Solid computer skills
Prior experience with EMR preferred
