📋 Description

• Effectively deliver on all requirements of the Clinical Roster including: Monitor and triage the
• Complete clinical assessments and reviews (e.g., VATs, CNAs)
• Support customers and families with informed decision-making
• Guide palliative care and advance care planning
• Respond to incidents and clinical escalations
• Contribute to continuous improvement and clinical quality initiatives

🎯 Requirements

• AHPRA Registration as a Registered Nurse
• First Aid certification
• Experience in Home and Community Care, ideally with aged care clients
• Strong clinical decision-making and case management skills
• A warm, human approach and genuine passion for empowering others
• Excellent communication and customer service skills
